Once I have submitted my booking, I understand that the personal details provided on this form and my patient record will be stored electronically and accessed only for clinical purposes, including appointment booking and reminders.

I consent to receiving treatment at Elite Performance Physio MCR upon agreement of the proposed treatment plan. I will inform the practitioner responsible for my care of any changes in my medical condition. Failure to do so will result in my taking full responsibility for any circumstances that might arise as a result of this change.

I have the right to withdraw this consent at any time during my visit and will inform the practitioner if this is the case.

I give permission for details of my treatment to be discussed with my GP and any insurance companies funding my physiotherapy treatment, if required.

I understand that I must give the clinic 24 hours’ notice if I wish to cancel my appointment without charge. Failure to do so will result in a request for payment in full. Failure to attend an appointment without prior notice will also result in a request for payment in full.
